About the Employee Rewards Program

Our Employee Rewards Program is designed to recognize and celebrate your hard work and dedication. By consistently showing up on time, maintaining excellent attendance, and contributing to a positive workplace, you earn points that can be redeemed for exciting rewards. From gift cards and company-branded merchandise to high-value electronics and wellness experiences, there's something for everyone. Simply accumulate points through your daily efforts and redeem them for the rewards that matter most to you. Employee Incentive Point Program

Applies To: All Staff (Excluding Agency Staff)

 

1. Purpose

The Employee Incentive Point Program is designed to recognize and reward employees for their dedication and commitment to maintaining consistent attendance and punctuality. This program aims to foster a culture of reliability, accountability, and engagement within the workplace.

 

2. Eligibility

All full-time and part-time employees that are employed by the parent Company Brighton Healthcare Group, excluding agency staff, are eligible to participate in the Employee Incentive Point Program. 

 

3. Program Structure

A. Earning Points Employees will earn points based on the following criteria:

  • Employees who arrive on time and do not miss a scheduled shift without using an excused absence (e.g., approved PTO, bereavement leave, or medical leave) will earn 2 points per week automatically.

  • Volunteering for extra shifts during critical staffing periods: 5 points per shift. Or as needed by the Admin.

  • Becoming a facility staff member and not agency: 100 points plus the sign on bonus

  • Receiving positive feedback from residents, families, or supervisors: 1 point.

  • Leaving a positive google review: 1 point per review

  • Leaving a positive review on any platform: 1 point per review 

  • Referring a successful new hire who completes the probationary period: 50 points per referral.

  • Demonstrating exceptional teamwork and leadership, recognized by management: 5 points per instance

  • Completing professional development or training courses approved by management: 10 points per course. (This can only be for courses that are not mandated for your position under our current CMS guidelines)

  • FOR SALARIED EMPLOYEES- Working outside of salaried hours: 1 point per occurrence (If you work three times on a specific day outside of your salaried hours it only counts as 1 occurrence, you have the potential to earn 7 points per week max) This will be verified every time.

  • FOR SALARIED EMPLOYEES- Offering to help and helping in other departments outside of your department: 5 points per occurrence (i.e. if an employee who works in the business office is a CNA and offers and helps on the floor during a busy time)

  • Administrators can gift an employee a gift card of 50 points, administrators can give three gift cards per month and an employee can only receive one gift card per quarter. 

B. Tracking and Accumulation

  • Points will be submitted by each employee and are subject to audit and review.

  • Employees can check their point balance through this website by logging into their account.

  • Employees will only accumulate points if they fill out the point calculator and participate in activities that accumulate points. 

 

4. Redemption of Points

Employees can redeem their accumulated points for prizes based on the following reward tiers: Full catalog of item possibilities listed below and in our Tier catalogs above items are subject to change 

 

Tier 1 – 50 to 250 Points

Gift Cards (e.g., coffee shops, restaurants, retail stores)

Company-branded merchandise (e.g., t-shirts, mugs, tote bags)

 

Tier 2 – 251 to 500 Points

Electronics (e.g., headphones, smart speakers)

 

Tier 3 – 501 to 1000 Points

High-value gift cards (e.g., grocery stores, gas cards)

Wellness packages (e.g., gym memberships, massage vouchers)

 

6. Program Guidelines and Restrictions

  • Points are non-transferable and cannot be shared or combined with other employees.

  • Unused points can be rolled over up to 500 annually and do not expire.

  • Points earned are subject to verification by management and may be adjusted in cases of discrepancies or errors.

  • Participation in the program is voluntary and subject to the organization’s discretion.

  • Submitting points will be the responsibility of the employee through the JotForm website. It will then be reviewed monthly. Any discovery of non truthful point submissions will result in that employees immediate removal from the program. 

 

7. Compliance and Program Review 

The Employee Incentive Point Program will be reviewed annually to assess its effectiveness and make improvements as necessary. Any modifications to the program will be communicated to employees in advance.

5. Streaming service subscriptions (3 months)

6. Professional development course reimbursement

7. Car detailing gift cards

8. Specialty kitchen gadgets (e.g., air fryer, blender)

9. Home fitness equipment (e.g., yoga mats, weights)

10. High-quality travel mugs or thermoses

11. Personalized company jackets or hoodies

12. Wellness app subscriptions (e.g., meditation apps)

13.Custom desk accessories (ergonomic mouse pads, etc.)

14.Charitable donation in employee's name

15. High-end personal care items (electric toothbrush, etc.)

16. Noise-canceling sleep masks

17. Hobby-related gift cards (craft stores, bookstores)

18. Subscription boxes (e.g., snacks, wellness, hobbies)

19. Foot massager or heating pad

Tier 3 – 501 to 1000 Points.

1. Visa Gift card

2. High-value grocery store gift cards

4. Wellness packages (gym memberships, massage vouchers)

5. Hotel staycation packages (local area hotels)

6. Smartwatches or fitness trackers

7. Tablet devices (basic models)

8. High-end noise-canceling headphones

9. Annual streaming service subscriptions

10. Weekend getaway packages (subject to availability)
